To prevent the spread of COVID-19 in our community, please note:

• If you are required to self-isolate, please inform us and we will reschedule your collection time or provide assistance in accessing our collections online.
• If you feel unwell, or have been unwell in the last week, we kindly ask you to reschedule your collection time to when you are feeling well again.
• We warmly welcome you to access the Click and Collect service, but please understand that we will practise social distancing of 1.5 metres.
• By submitting this visitor declaration form I confirm that the information given is true, complete and accurate.
